Architecture decisions

A decision record is written once and then left alone. When a later decision changes the answer, it supersedes the record rather than editing it, so the reasoning that was true at the time stays readable.

Numbering is not contiguous. A gap means a number was allocated to a decision that was never recorded, not that a record is missing from this list.
